name it ("new client intake", "pet info", "design brief") and pick a url slug — that's the public link customers will get.

add fields
10 live field kinds: short text, long text, email, phone, number, date, dropdown, checkboxes, yes/no, 1–5 rating. drop them in, set labels, mark required if needed.

preview, then publish
save as a draft, preview from your phone to make sure it reads clean, then click publish. the share link goes live the moment you do.
tipshorter forms get way more completions. a 6-question form lands replies; a 20-question one gets abandoned. cut anything you don't truly need before the appointment.
